Newborn Successfully Treated for Rare Intra-abdominal Pulmonary Sequestration

A 30-day-old newborn has made a remarkable recovery after undergoing surgery to treat an extremely rare condition known as intra-abdominal extralobar pulmonary sequestration (PS). This case, reported in a recent medical journal, highlights the successful application of modern medical techniques in managing such complex cases.

Symptoms and Diagnosis

Common symptoms of intra-abdominal extralobar pulmonary sequestration in neonates typically include respiratory distress, although some cases may be asymptomatic and detected incidentally by imaging. In this case, the patient had a history of recurrent left-sided pleural effusion. A CT scan with venous contrast identified an intrabdominal extralobar pulmonary sequestration on the left, associated with a diaphragmatic hernia.

Diagnostic methods used primarily involve prenatal and postnatal imaging techniques. Prenatal sonographic detection is a key method for identifying intra-abdominal extralobar pulmonary sequestration before birth. Postnatal imaging includes ultrasound, MRI, and contrast-enhanced CT. Contrast-enhanced CT can identify the systemic arterial supply, a hallmark of pulmonary sequestration.

Treatment and Recovery

The patient underwent left thoracotomy with excision of the accessory lung. The artery feeding the PS was embolized to reduce bleeding, and the accessory lung was devascularized during the embolization procedure. The left diaphragm was reconstructed using bovine pericardium.

Following the surgery, the patient remained asymptomatic and showed appropriate development for his age at 8 months of follow-up. He was discharged from the hospital after 4 days without complications.

Pulmonary sequestration is a congenital anomaly characterized by a mass of nonfunctioning lung parenchyma. It receives a systemic arterial blood supply, which differentiates it from normal lung tissue. Intra-abdominal pulmonary sequestration is a rare subtype.
